UNDERSTANDING THE MEDICARE PART D PRESCRIPTION PAYMENT PLAN
No one likes the feeling of dread that can arise at the beginning of the year when you visit your pharmacy in January, when you know the costs of your medications and your Part D plan has increased its rates from last year. This really can be a stressful experience for many senior Medicare beneficiaries. But a new option unveiled a few years ago may be something you haven't heard of. This option is called the Medicare Prescription Payment Plan, or MPPP.
The MPPP is a voluntary option you can choose to enroll in. The MPPP changes how you pay for prescription medication by spreading costs into more consistent, regular monthly payments that are easier to manage.
In this article, you will learn and understand what the Medicare Prescription Payment Plan (MPPP) is, how it works, who it may benefit, and how to enroll in the plan. As a Medicare broker for the past 11 years and a Certified Medicare Planner (CMP), I wanted to share this information and see if it might benefit you or someone you know who struggles to keep up with prescription medications.
DEFINING THE MEDICARE PART D PRESCRIPTION PAYMENT PLAN (MPPP)
The Medicare Part D Prescription Payment Plan model was introduced under the Inflation Reduction Act (IRA). The MPPP is a voluntary payment option for any/every Medicare Part D beneficiary. The PPP works with both Medicare Advantage plans (Medicare Part C) and stand-alone Medicare Part D plans.
Instead of you having to pay the full amount of your Medicare Part D deductible, co-pays, and/or coinsurance directly to the pharmacy when you pick up your medicine, the Medicare Part D Prescription Payment Plan will allow you to spread the total drug costs for your prescription medications into predictable, monthly costs throughout the year.
Note: This program is not a discount program or financial aid. It does not lower your total drug costs or save you money on your medications. Instead, it changes when you pay, acting as a budget-smoothing tool.
WHY THE MEDICARE PART D PRESCRIPTION PAYMENT PLAN IS A GOOD IDEA
NO EXTRA INTEREST OR COST BY SPLITTING COSTS: There is absolutely no fee to join the Medicare Prescription Payment Plan, and your monthly installments carry a 0% interest rate.
YOU WON’T HAVE TO PAY AT THE PHARMACY: Once you are enrolled, your Part D plan communicates with your pharmacy. When you go to pick up a covered drug, you will walk away paying $0 at the counter. Instead, you will receive a monthly bill from your insurance plan.
Works Side-by-Side with the Medicare Cap: Separate from this payment plan, federal law has permanently capped total annual out-of-pocket Part D drug costs for 2027 at $2,400. That is up $300 from the 2026 amount, but it is still a much better alternative than the old Medicare Part D “DONUT HOLE”. In 2027, the most you will pay for your prescriptions for the entire year is $2,400.
HOW DOES THE BILLING PROCESS WORK IF YOU ARE INTERESTED IN THE MEDICARE PART D PPP?
Most people assume that you can just divide your January Prescription Drug Total by 12 and call it a day. However, Medicare has developed guidelines and shared them with each Medicare Part D Insurance company that offers prescription drug coverage to Medicare beneficiaries.
That is why your specific Part D plan uses a specific formula to determine your costs. Your bill is determined by taking your total prescription costs (plus any leftover balance from the previous month) and dividing it by the number of months left in the calendar year.
As a result, your monthly bill can change. If you fill a high-cost specialty medication early in the year, your costs are spread over many months, which makes your monthly costs more predictable and easier to budget for.
IS IT BENEFICIAL TO ENROLL IN THE MEDICARE PART D PRESCRIPTION PAYMENT PLAN?
It is a good time to remind everyone with Medicare that your enrollment in the Medicare Part D Prescription Payment Plan is completely voluntary. You don't have to do this. To help, please review our guidelines to see whether any of the scenarios below apply to you and your Prescription needs.

You have been prescribed high-cost/high-tiered prescriptions for things such as Cancer protocols, Rheumatoid Arthritis, Multiple Sclerosis, or if you have several Tier 3 brand-name medications that you take daily.
You plan on taking a big hit in drug costs at the beginning of the year before you can meet your plan's deductible threshold.
If you are on a fixed/limited income and find it difficult to manage your out-of-pocket expenses.

Your overall yearly drug costs are relatively low.
Your monthly medication costs are already steady and predictable.
You qualify for Medicare’s Extra Help program or a Medicare Savings Program, which already significantly lowers your out-of-pocket costs.
ENROLLING IN THE MEDICARE PART D PRESCRIPTION PAYMENT PLAN
You can opt for the Medicare Prescription Payment Plan at any point during the calendar year. To sign up, you must contact your specific Medicare Part D or Medicare Advantage plan provider directly. However, you cannot sign up for this at the Pharmacy; you must enroll in the MPPP using one of the methods below.

ONLINE: Log in to your health plan’s online member portal.
OVER THE PHONE: Call the customer service number on the back of your insurance card.
ENROLLING BY MAIL: Download and submit a paper election form from your provider’s website.
Once you are enrolled, if you stay with the same insurance plan next year, your participation will automatically renew, ensuring your medication expenses remain predictable year after year.
